Support Coordinator

This is an exciting opportunity to contribute to the delivery of meaningful support services in remote Aboriginal communities.

As a Support Coordinator, you will play a key role in providing coordinated support to individuals with complex needs. You will work closely with a close-knit team to develop and implement trauma-informed, culturally respectful plans that meet the unique needs of each individual.

  • Key Responsibilities:
  • Coordinate support services for individuals with complex needs
  • Develop and implement trauma-informed, culturally respectful plans
  • Work collaboratively with colleagues to achieve shared goals
  • Maintain accurate records and reports as required

Requirements:

  • Qualifications:
  • Minimum Certificate III in Disability or equivalent
  • Experience in Human Services or Disability Services
  • Strong trauma-informed practice and cultural awareness
