  • Coordinate care for patients, including scheduling appointments and arranging for diagnostic tests.
  • Communicate with healthcare providers to ensure timely follow-up and coordination of care.
  • Educate patients and families on healthcare services and treatment plans, including information and referrals to community resources.
  • Assist patients in navigating healthcare systems and understanding their treatment plans.
  • Collaborate with insurance providers to ensure proper coverage for patient care.
  • Participate in quality improvement activities to improve patient care.
  • Advocate for clients' rights and needs and work to address any issues that may arise.
  • Work in partnership with diverse professionals to deliver seamless and well-rounded care to clients.
  • Maintain accurate and up-to-date client records and report any changes or concerns to relevant parties.
  • Participate in continuing education and training opportunities to maintain knowledge and skills.
Requirements
  • Minimum 2 years of experience in a healthcare setting - at least 1 year experience as a Care Coordinator.
  • Knowledge of healthcare services and medical terminology.
  • Excellent communication and interpersonal skills.
  • Strong organisational skills and attention to detail.
  • Ability to work in a fast-paced environment.
  • Proficient in Microsoft Office and electronic medical record systems.
  • Ability to work independently and as part of a team.
  • Understanding of relevant laws, regulations, and community resources is desirable.
