Job Title: Aboriginal Health Navigator

About the Role:

Our organization is seeking a skilled and experienced Aboriginal Health Navigator to support children and young people in out-of-home care. As a key member of our multidisciplinary team, you will work collaboratively with local healthcare providers to help clients navigate health services and overcome barriers to access.

Key Responsibilities:
  • Identify and address barriers to healthcare access for vulnerable populations.
  • Develop and implement effective strategies to engage clients and their families in health service delivery.
  • Work closely with multidisciplinary teams to provide expert input on First Nations health issues.
  • Make informed referrals based on assessment outcomes and support clients' access to relevant services.
Requirements:
  • Hold knowledge and experience working within the local Aboriginal community and culture.
  • Effectively communicate and collaborate with diverse stakeholders, including healthcare providers, families, and community members.
  • Respect the rights of clients to informed choice and independence.
  • Maintain confidentiality, privacy, and professional boundaries.
  • Be self-motivated, able to work independently, and contribute to the success of the team.
