Job Title: Aboriginal Health Navigator
About the Role:Our organization is seeking a skilled and experienced Aboriginal Health Navigator to support children and young people in out-of-home care. As a key member of our multidisciplinary team, you will work collaboratively with local healthcare providers to help clients navigate health services and overcome barriers to access.
Key Responsibilities:- Identify and address barriers to healthcare access for vulnerable populations.
- Develop and implement effective strategies to engage clients and their families in health service delivery.
- Work closely with multidisciplinary teams to provide expert input on First Nations health issues.
- Make informed referrals based on assessment outcomes and support clients' access to relevant services.
- Hold knowledge and experience working within the local Aboriginal community and culture.
- Effectively communicate and collaborate with diverse stakeholders, including healthcare providers, families, and community members.
- Respect the rights of clients to informed choice and independence.
- Maintain confidentiality, privacy, and professional boundaries.
- Be self-motivated, able to work independently, and contribute to the success of the team.
