The Care Coordinator role is a vital position within a multidisciplinary team, responsible for coordinating patient assessments from reception to discharge planning. This key function will also facilitate the development of Integrated Care Plans for patients with complex health needs.
- Coordinate patient assessments and develop integrated care plans in collaboration with multidisciplinary teams and external service providers.
- Liaise with other healthcare teams and external service providers to ensure seamless care coordination.
- Contribute to team-based care plans and document interventions and plans.
- Ensure patients are actively engaged with their GP or community health supports on discharge.
