About the Role:We are seeking a highly skilled Aboriginal Health Navigator to join our multidisciplinary health service. Our team supports children and young people in out-of-home care, providing tailored healthcare solutions.
Main Responsibilities:- Identify and address barriers to healthcare access for children and young people, ensuring equitable outcomes.
- Collaborate with external services, including dental, maternal and child health, and child protection agencies.
- Participate in multidisciplinary assessments, providing expert input on First Nations children and young people's needs.
- Make informed referrals based on assessment outcomes, facilitating access to relevant services.
- Demonstrated knowledge of and experience within local Aboriginal communities and cultures.
- Excellent interpersonal skills, enabling effective collaboration and communication with clients, families, and external agencies.
- Awareness of the rights of children and young people to informed choice and independence, ensuring respectful and empowering practice.
- Ability to maintain confidentiality, privacy, and effective communication throughout interactions.
- Self-motivated, able to work independently, and eager to contribute to a collaborative team environment.
