Job Role: Pre-Assessment Officer

About the Position

We are seeking an experienced professional to work within our Gateway program at a leading health service. The Gateway program serves as the primary entry point for Allied Health and Community Services referrals.

  • Process new client referrals with accuracy and efficiency.
  • Register all clients onto our data management systems.
  • Carry out initial needs assessments.
  • Conduct home risk screening evaluations.
  • Book appointments for clients as needed.
  • Foster effective communication and collaboration with Allied Health, Community, and Nursing staff members.

Essential Skills and Qualifications:

  • Demonstrate excellent interpersonal and communication skills.
  • Possess strong organisational, time management, and problem-solving abilities.
  • Display proficient computer and literacy skills.
  • A Certificate III in Allied Health Assistance or equivalent qualification is desirable but not essential.
