Job Role: Pre-Assessment Officer
About the PositionWe are seeking an experienced professional to work within our Gateway program at a leading health service. The Gateway program serves as the primary entry point for Allied Health and Community Services referrals.
- Process new client referrals with accuracy and efficiency.
- Register all clients onto our data management systems.
- Carry out initial needs assessments.
- Conduct home risk screening evaluations.
- Book appointments for clients as needed.
- Foster effective communication and collaboration with Allied Health, Community, and Nursing staff members.
Essential Skills and Qualifications:
- Demonstrate excellent interpersonal and communication skills.
- Possess strong organisational, time management, and problem-solving abilities.
- Display proficient computer and literacy skills.
- A Certificate III in Allied Health Assistance or equivalent qualification is desirable but not essential.
