Job Title: Care Coordinator
About the RoleWe are seeking a dedicated and experienced healthcare professional to join our team as a Care Coordinator. The successful candidate will be responsible for coordinating patient care, providing self-management support, and navigating the healthcare system.
Key Responsibilities- Conduct comprehensive patient assessments
- Develop and implement personalized care plans
- Coordinate patient healthcare needs across the continuum
To be successful in this role, you will need:
- An appropriate qualification in nursing or allied health and eligibility for registration with the relevant body
- At least 5 years of acute clinical experience
- Experience in assessment, discharge planning, and care planning
