**Role Overview**:
We are seeking a compassionate and organized Support Coordinator to join our

overseeing the delivery of support services to our clients, ensuring their needs are

met with the highest standards of care and professionalism.

**Key Responsibilities**:

- Coordinate and manage individual support plans for clients, ensuring they receive

appropriate services tailored to their needs.
- Liaise with clients, their families, and other professionals to assess needs and

develop comprehensive support plans.
- Monitor and review support plans regularly to ensure they are effective and up-to
- date.
- Provide guidance and support to support workers, ensuring they are delivering

services in line with company policies and procedures.
- Maintain accurate records and documentation, ensuring compliance with relevant

regulations and standards.
- Respond to emergencies and provide crisis intervention as needed.
- Participate in staff training and development programs to enhance skills and

knowledge.
- Foster a positive and inclusive environment for both clients and staff.

**Qualifications and Experience**:

- A relevant qualification in social work, psychology, disability support, or a related

field.
- Previous experience in a similar role within the disability or social care sector.
- Excellent communication and interpersonal skills.
- Strong organizational and time management abilities.
- A compassionate and patient approach to working with individuals with disabilities.
- Ability to work independently and as part of a team.
- Knowledge of relevant legislation and best practices in disability support.
- Proficiency in using computer software for record-keeping and communication.
