We are seeking a motivated **Case Manager** to design and deliver high-quality, person-centered care that helps clients remain independent and engaged in their communities.

**Key Responsibilities**:

- Provide culturally and linguistically appropriate aged care services aimed at optimizing health, wellbeing, and reablement.
- Design and manage individualized care packages based on client goals, including assessments, referrals, and risk evaluations.
- Support clients to live independently at home, coordinating services and field staff as needed.
- Foster strong relationships with clients, families, and service providers, ensuring exceptional customer service.
- Maintain accurate documentation and comply with all relevant aged care standards and legislation.
- Actively contribute to continuous improvement and support Work Health and Safety practices.

***Essential Criteria**:

- Degree qualification in Social Work, Nursing, Allied Health, or a related field.
- Experience in case management with knowledge of aged care and related health issues.
- Strong communication, time management, and problem-solving skills.
- Demonstrated knowledge of Work Health and Safety requirements.
- Current National Police Certificate and Driver’s License.

***Desirable Criteria**:

- Experience in an aged care environment and familiarity with regulatory frameworks.
- Ability to speak Italian or experience working with culturally diverse communities.
