Empowering Community Support Leader

We are seeking a highly skilled and passionate Senior Support Worker to join our team as a Community Support Leader.
About YouYou will be a driven and passionate individual who is committed to empowering people with disabilities to improve their well-being, quality of life, and community connections.
You will have a minimum of two years of experience in working with people with intellectual or other disabilities and their families, and extensive experience in supporting individuals with complex healthcare needs.
You will possess excellent interpersonal and communication skills, written and verbal, and be able to prioritise tasks and adopt a flexible approach to working methods and time.
You will also have a Certificate IV specialising in Disability Studies and/or Human Services and/or substantial experience in this or a similar role, as well as experience supporting or coaching support staff in community settings.
Responsibilities- Provide direct support to individuals who have contracted HomePlace as their provider.
- Facilitate connections and relationships that assist in creating social and economic participation for the person being supported.
- Mentor and support team members.
- Oversee the day-to-day management of the supports in the person's home.
- Facilitate team meetings and effective communication.
- Provide person-centred active support with participants over a 7-day weekly schedule which will include early mornings, evenings, weekends, and sleepovers.
- A minimum of two years of experience in working with people with intellectual or other disabilities and their families.
- Extensive experience in supporting individuals with complex healthcare needs.
- Confidence in assisting with behavioural challenges.
- Proven experience in implementing Positive Behaviour Support (PBS) strategies to enhance participant well-being and outcomes.
- Certificate IV specialising in Disability Studies and/or Human Services and/or substantial experience in this or a similar role.
- Experience supporting or coaching support staff in community settings.
- Continuous improvement methodology.
- Experience with project management.
- Excellent interpersonal and communication skills, written and verbal.
- Good organisational and time management skills with the ability to work using your own initiative.
- Ability to prioritise tasks and adopt a flexible approach to working methods and time.
- Ability to enthuse others and be a good team player.
- Proficiency in the use of IT, in particular MS Office.
- Current NDIS Clearance.
- Senior First Aid / Basic Emergency Life Support, and CPR certificate.
- Hold a current driver's license, have access to a vehicle, and be willing to drive to undertake HomePlace business.
- COVID Vaccinated x 3.
