Bene Aged Care provides quality residential and community home care to over 2,500 older people throughout South Australia. We have a workforce of approximately 630 employees across 6 sites and in the community, recognized for our exceptional person-centred care.

**About the Role**

We are looking for a compassionate and experienced Case Manager to join our Community Services team. This role involves designing and managing individualized care packages to help clients maintain their independence and active participation in the community.

**Your part in Bene Aged Care**:
**Key Responsibilities**
- Conduct comprehensive assessments of clients' needs, considering physical, emotional, and social factors.
- Advocate for clients' rights and needs within the healthcare system.
- Develop individualized care plans in collaboration with clients, their families, and healthcare professionals.
- Maintain effective relationships and provide support and guidance to clients, families, and community organizations.
- Foster effective communication among interdisciplinary teams to ensure holistic and seamless care delivery.
- Maintain accurate and up-to-date client records, documenting assessments, care plans, and progress notes.
- Prepare detailed reports for internal and external stakeholders as required.
- Support clients to live independently at home.
- Design and manage individual care packages.
- Deliver culturally and linguistically appropriate aged care services.
- Market and promote Bene Aged Care’s services.
- Deliver services according to guidelines and agreements.
- Contribute to continuous improvement initiatives.

**What makes a difference for us**
- Relevant degree in Social Work, Allied Health, Mental Health, Nursing, or Disability Services.
- Comprehensive case management knowledge, especially with aged-related health issues.
- High level of written and oral communication skills and computer literacy.
- Ability to design and implement case management strategies.
- Experience in an Aged Care Environment and understanding of its regulatory framework.
- Italian speaking or experience working with CALD communities. (desirable)
- Knowledge of the Aged Care Industry and WHS requirements.
- Current National Police Certificate (or ability to obtain at Bene’s expense).
- Valid Driver’s License.
