Why this role matters
McCombe House is a 6 - 8 week safe and secure crisis accommodation for women and children from all cultures and backgrounds. McCombe House provides supported emergency accommodation for single women and women with children escaping domestic violence and homelessness in Hobart, Tasmania.
About the role
We are seeking a values-driven and proactive Case Manager to provide in house and outreach case management support to women and women with children who have experienced and/ or currently experiencing family violence. The role also encompasses the delivery of assessment and crisis response.
Reporting to the Team Leader/Manager, this is a maximum term (until September 2025), part-time (28 hours per week) position based in Hobart, Tasmania. Salary and conditions are in accordance with the SCHADS Award Level 5.
You will successfully- Establish relationships with clients that encourage self-determination and participation.- Provide Case management support to clients.- Make ongoing assessment of the client’s issues, needs and circumstances from initial intake to exit.- Assist clients to access support services and establish community supports, and where needed are assisted through advocacy with service providers.- Monitor regularly and review the progress and outcomes of individual case plans and where needed action is taken to ensure the plan continues to meet client needs or goals.- Identify risks to clients, staff and The Salvation Army and bring them to the attention of the Team Leader as soon as possible.- Identify service gaps and make recommendations for service delivery improvements to the Team Leader.
You will have- A degree qualification in social work, welfare, community development or related field OR demonstrated high level of sector knowledge and skills attained through previous appointments, service, and study/development.- A NSW Employee Working with Children Check.- A current First Aid Certificate.- A valid NSW Driver Licence.- Relevant experience in a social service environment.
