About the role
We are seeking a dedicated Family Violence Case Manager to work with Women and Women with Children to develop client led case plans that best interest of clients and children. You will provide in house and outreach case management support to those who have experienced and/or are currently experiencing family violence. This role will be instrumental in delivering high quality, safe and effective family violence and child wellbeing responses to those seek support, safety, and recovery.
Responsibilities include
- Case management support is provided to clients in accordance with TSA case management standards of practice
- Ongoing assessment is made of the client’s issues, needs and circumstances from initial intake to exit
- Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
- Case plans are implemented with active client participation
- Risks to clients, staff and TSA are identified and brought to the attention of the Team Leader
This is a permanent part-time position, 28 hours per week (4 days per week) 7 hour shift per day. This is based in McCombe House, located in Hobart, TAS. Salary and conditions are in accordance with SCHADS Level 5.
What you will have
- Degree qualification in Social Work, Welfare, Community Development or related field OR demonstrated high level of sector knowledge and skills attained through previous appointments, service, and study/development
- Relevant experience in a Social Service environment [add sector if appropriate e.g., AOD]
- Act in line with TSA mission, values and code of conduct and industry standards
- National Police Record Check required
- Employee Working with Vulnerable Person's Check required
- First Aid required
- Driver’s Licence required
