Support the daily HMIS responsibilities of the Quality Improvement Department through timely and efficient data entry of client services and records in Evolv and Clarity.

Essential Job Functions

  • Ensures that client data is entered into required databases (Evolv, Clarity) timely, efficiently and accurately.
  • Enters and maintains service information in Clarity by reviewing and entering data for new and existing clients including entry/exit, referrals, re-certification, transfer of services, annual assessments and service outcomes.
  • Scans, uploads, and attaches all paper documents to the client record in Evolv
  • Communicates with Manager of HMIS and program staff/management regarding inaccurate information or null values on Evolv intake/update/discharge documents
  • Follows-up with program staff/management on all intakes/updates/discharges until all information is accurately entered into Clarity
  • Ensures confidentiality of client-related and other clinical information in accordance with established guidelines and procedures.

  • Consistently interacts with members of this and other organizations in a respectful, professional manner; productively contributes to the functioning of an interdisciplinary team.
  • Assumes additional responsibilities and performs special projects as needed or directed.

Required Qualifications

Education, Training and/or Experience

High school diploma or equivalent with a minimum of 3 years of data entry experience.

Certifications, Licenses, Registrations

Non-violent crisis intervention training (Prevention First for non-client-facing staff) (must be completed within 60 days of hire).Seniority level
