The key responsibility of the HMIS Specialist is to provide timely and efficient data entry support to the Quality Improvement Department through the use of Evolv and Clarity.

Key Responsibilities:
  • Ensures accurate and timely data entry into required databases (Evolv, Clarity) for client services and records.
  • Reviews and enters service information in Clarity for new and existing clients, including entry/exit, referrals, re-certification, transfer of services, annual assessments, and service outcomes.
  • Scans, uploads, and attaches all paper documents to the client record in Evolv.
  • Communicates with Manager of HMIS and program staff regarding inaccurate information or null values on Evolv intake/update/discharge documents.
  • Follows-up with program staff until all information is accurately entered into Clarity.
  • Ensures confidentiality of client-related and other clinical information in accordance with established guidelines and procedures.
Additional Duties:
  • Adheres to ethical, confidentiality, and professional standards requirements of federal and state law, organizational policies, and licensing and credentialing boards.
  • Speaks and acts in a manner sensitive and responsive to individuals' ethnic, cultural, and developmental backgrounds, as well as any disabling conditions.
  • Interacts with colleagues from this and other organizations in a respectful, professional manner, contributing productively to an interdisciplinary team.
