Support the daily HMIS responsibilities of the Quality Improvement Department through timely and efficient data entry of client services and records in Evolv and Clarity.
Essential Job Functions Ensures that client data is entered into required databases (Evolv, Clarity) timely, efficiently and accurately.
Enters and maintains service information in Clarity by reviewing and entering data for new and existing clients including entry / exit, referrals, re-certification, transfer of services, annual assessments and service outcomes.
Scans, uploads, and attaches all paper documents to the client record in Evolv Communicates with Manager of HMIS and program staff / management regarding inaccurate information or null values on Evolv intake / update / discharge documents Follows-up with program staff / management on all intakes / updates / discharges until all information is accurately entered into Clarity Ensures confidentiality of client-related and other clinical information in accordance with established guidelines and procedures.
Additional Duties And Responsibilities Adheres to the ethical, confidentiality, and professional-standards requirements of federal and state law, the policies of this organization, and the policies of licensing and credentialing boards.
Speaks and acts in a manner that is sensitive and responsive to a person's ethnic, cultural, and developmental background, to any disabling conditions, and to conditions that may affect the person's ability to form accurate perceptions, process information, understand directions, and communicate important information.
Consistently interacts with members of this and other organizations in a respectful, professional manner; productively contributes to the functioning of an interdisciplinary team.
Assumes additional responsibilities and performs special projects as needed or directed.
Required Qualifications Education, Training and / or Experience High school diploma or equivalent with a minimum of 3 years of data entry experience.
Certifications, Licenses, Registrations Non-violent crisis intervention training (Prevention First for non-client-facing staff) (must be completed within 60 days of hire).
