Job summary

Infant Child and Family Health and Wellbeing Hub


The Infant Child and Family Health and Wellbeing Hub is a new service that will provide therapy that considers all aspects of an infant's or child's life.

These services will provide wellbeing support, peer support and care coordination to help with different kinds of challenges including developmental, behavioural, and mental health concerns.

This service has funding for community health, mental health, and family support services. The Infant, Child and Family Health and Wellbeing Hub will sit alongside ELHMS - Monash Childrens and the Child and Family Program - Monash Health Community


As this is a new service, as part of the recommendations of the Mental Health Royal Commission, there will be two phases of service delivery.

The first phase will be an upstanding of an interim model of care taken from the needs of both existing mental health and community health paediatric services within Monash Health that meet the objectives of the Hub.

The second phase will run in parallel to phase one and will consist of a new codesigned model of care for the ongoing service delivery of the Hub.

Within the first 12 months there will be time allocated to workshopping the new model of care with other team members, consumers, and stakeholders.

This role is for the Community Health Manager within the Infant Child and Family Health and Wellbeing Hub.

The Community Manager and the Mental Health Manager will be responsible for managing the Hub's business within Monash Health, supporting a multidisciplinary team to deliver high-quality care.

Both managers will have a split role, consisting of shared operational governance and clinical load.

The manager(s) are responsible for leading the current services, understanding the developmental and mental health landscape, and seeking to grow and remodel the service to meet population needs.

The managers will be required to build responsive policies, procedures and processes for the service and to ensure staff are provided with high-quality supervision and professional development.
