The Resilient and Connected Communities department proactively looks at our community's needs now and into the future, planning through policy, advocacy, programming, engagement and partnerships. Developing and leading an integrated approach to delivering community impact and leading the development of practices that support the achievement of social value and community outcomes. Building and strengthening community resilience through planning and responsive management of community emergencies and impacts.

The Community Connections team build community strengthening and connections with a specific focus on age friendly communities as well as supporting community groups and organisations through community grants and volunteering.

Embedding this with the new Resilient and Connected Communities department to ensure emerging local knowledge, evidence and data are linked to drive evidence-based decision making, measurable outcomes and support community resilience.



About the Role:

The Service Navigation and Assessment Lead works closely with the Community Connections Coordinator to provide leadership to the Regional Assessment Service (RAS) to meet strategic key outcomes as identified in the Banyule City Council Plan and Banyule's Older Adults Strategic Plan, and aligned with the State Government guidelines. Under the guidance of the Community Connections Coordinator, the occupant of this position will participate in the development of and lead the Regional Assessment Service as well as develop and deliver Service Navigation for people who are already or are moving into the ageing cohort. The RAS program provides a person
-
centered focus on individuals as they access the aged service system and ensure that support referrals are tailored to meet the needs of individuals in Banyule.**
The Service Navigation and Assessment Lead will:

  • Manage the day

  • day support, leadership and supervision of the Regional Assessment Service:

  • Undertake a Service Navigation program in relevant aspects of Council's delivery of service and support to older people:

Strategically manage financial and physical resources, as well as policies and procedures to ensure the effective delivery of programs and activities within a strong operational, financial and strategic framework:


  • Work with the Coordinator and Manager to understand and navigate service and legislative changes in Aged Assessment and the revised assessment program through the Support at Home Policy Initiative (Commonwealth):

Ensure that all assessment activities uphold a high standard of safety and inclusion in all programming and service delivery:


  • Ensure all reporting requirements are delivered in a timely manner

Skills, experience and certificates:

Qualifications in Social Work/Human Services/Allied Health/Social Sciences and demonstrated experience in working with older people at an appropriate level to meet the requirements of the position:


  • Demonstrated experience in assessment and aged care planning and support:
-
Demonstrated leadership abilities coming with successful experience in managing staff through a change process:


  • Demonstrated financial management, and extensive experience in delivering State Government and organisational reporting requirements:
-
Demonstrated experience in strategic planning, policy development and planning in a human services context:


  • Extensive knowledge, skills and experience in policy, strategy, and plan development relevant to the Aged and Disability Services Sector:

Highly developed communication skills including negotiation skills, written communication skills and demonstrated experience in the preparation of reports and presentations:


  • High level advocacy and dispute resolution capabilities:
-
A commitment to the delivery of high-quality services and a proven record in achieving desired outcomes:


  • Current CPR/First Aid Certificate
