Join us at Hampstead Centre as we create a new centre of excellence in caring for older persons. This is an exciting opportunity to be part of a dynamic multidisciplinary team, helping to shape and implement an innovative model of care that will set new standards in patient outcomes.

Aligned with CALHN's strategic vision for delivering world-class healthcare, Hampstead Centre is launching a new program, “CO-ACT” (Care of the Older Person and Community Transition), focused on providing exceptional care for older adults transitioning from acute to ongoing care. As a member of our CO-ACT team, you'll work in a supportive, family-like environment where collaboration is key, and your input on improving systems and practices is highly valued.
