Care Coordinator

Clinical Nurse Specialists play a vital role in mental health care, providing support and guidance to children and young people.
This position offers an opportunity to work as part of a multidisciplinary team, providing high-level clinical coordination and specialist triage services. You will be responsible for conducting crisis and clinical assessments, developing care plans, and contributing to reducing Emergency Department waiting times.
A key aspect of this role is working with a collaborative team, utilising your expertise to provide treatment recommendations and specialist support to healthcare professionals, community organisations, and other stakeholders.
The successful candidate will have a passion for delivering person-centred care, ensuring the needs of individuals are met while promoting a positive environment. They will possess extensive experience as a registered nurse with tertiary qualifications in mental health/psychiatric nursing and current registration.
