Your opportunity
- Part-Time (4 days a week, Tuesday to Friday) - Based at Jannali_
In this rewarding part-time role, you'll provide specialist advice, advocacy, and practical support to people living with dementia and their carers. You will work 3 days in the South East Sydney Region and 1 day in the Illawarra Region, delivering individualised case management, facilitating education programs, and raising community awareness about dementia.
With empathy and expertise, you will:
- Provide person-centred case management that supports dignity, choice, and wellbeing- Deliver evidence-based advice and advocacy during home visits and group settings- Conduct educational talks to increase understanding of dementia in the community- Complete timely assessments and care plans to ensure services are optimised and effective- Build and maintain strong relationships with clients, families, carers, and stakeholders- Manage budgets, maintain accurate client records, and ensure compliance with organisational standards
What you'll bring
You're a compassionate and knowledgeable professional who is passionate about making life better for older people and their families.
You'll need:
- Diploma in Community Services, Dementia Care, or equivalent qualification (essential)- Strong understanding of dementia, its progression, and its impacts on individuals and carers- Experience working with older people, particularly those living with dementia- Excellent communication, presentation, and advocacy skills- Proficiency in MS Office and client database systems- Current C Class driver's licence and access to a reliable vehicle
Ideally, you'll also have:
- Experience in facilitating community education programs- Knowledge of aged care service systems and funding arrangements
