Job Opportunity

As a Dementia Care Adviser, you will provide specialist advice, advocacy and practical support to individuals living with dementia and their carers.

You will be based in a community setting, delivering individualised case management, facilitating education programs and raising awareness about dementia.

Key responsibilities include:

  • Providing person-centred case management that supports dignity choice and wellbeing.
  • Delivering evidence-based advice and advocacy during home visits and group settings.
  • Conducting educational talks to increase understanding of dementia in the community.
  • Completing timely assessments and care plans to ensure services are optimised and effective.
Requirements

To succeed in this role, you will need:

  • A Diploma in Community Services or equivalent qualification (essential).
  • A strong understanding of dementia, its progression and its impacts on individuals and carers.
  • Experience working with older people, particularly those living with dementia.
  • Excellent communication, presentation and advocacy skills.
