Job Title: Support Coordinator – NDIS (Contractor Role)

We are now seeking a Support Coordinator to join us on an as-needed, contractor basis ideal for someone looking to supplement existing work or pick up occasional referrals with flexibility and autonomy.

Key Responsibilities:

  • Support participants to understand and implement their NDIS plans.
  • Connect participants with services and monitor outcomes.
  • Coordinate service bookings and provider relationships.
  • Assist with documentation, progress reports, and review preparation.
  • Build strong, respectful relationships with participants and their networks.

Requirements:

  • Experience delivering Support Coordination under the NDIS.
  • Strong understanding of the NDIS framework and provider landscape.
  • ABN and contractor readiness.
  • NDIS Worker Screening Check.
  • National Police Clearance.
  • Excellent communication and organisation skills.
  • Relevant qualifications (e.g. Certificate IV or higher in Community Services, Mental Health, Disability – preferred).
