We are currently working with a respected provider of disability support services in Perth who is looking for a passionate and dedicated NDIS Coordinator to join their team.

**Role Overview**: As an NDIS Coordinator, you will play a pivotal role in ensuring that individuals with disabilities receive the support and care they deserve. You will work closely with clients to create tailored care plans, coordinate service delivery, and collaborate with multiple stakeholders to ensure high-quality outcomes.

**Key Responsibilities**:

- Develop and implement individualized care plans for clients in an NDIS setting.
- Coordinate services and support to ensure high-quality, timely care.
- Ability to assist in management of a team of Support workers and Carers in performing their roles.
- Communicate effectively with clients, families, and service providers to ensure the best possible outcomes.
- Monitor and assess client progress, making adjustments to care plans as necessary.
- Guide clients and families in navigating the NDIS system.
- Ensure compliance with NDIS guidelines and regulations.

**Key Requirements**:

- Previous experience in the disability sector, particularly in an NDIS setting (preferred but not required).
- Understanding of NDIS processes and guidelines.
- Excellent communication and problem-solving skills.
- Ability to work independently and collaboratively with a team.
- Compassionate and person-centered approach to service delivery.
- Previous experience in community nursing or therapy (highly desirable)
