About the Role

We are seeking an experienced and proactive 
NDIS
 
Care Coordinator – SIL & Community Operations
 to oversee daily service delivery, ensure compliance, and support staff across SIL homes and community programs.

You will be the 
central point of operational contact
, ensuring participants receive safe, consistent, and high-quality support - while risks, gaps, and incidents are identified and escalated and managed promptly to management and Directors.

Key Responsibilities

  • Oversee SIL and community operations across multiple participant sites.
  • Conduct 
    participant intakes
    , create 
    Service Agreements
    , and develop 
    individualised Support Plans
     aligned with NDIS goals.
  • Carry out 
    risk assessments
    , identify gaps in service delivery, and implement corrective actions.
  • Review 
    progress notes daily
     to ensure accuracy, quality, and timely action for follow-up needs.
  • Supervise, mentor, and monitor staff performance and adherence to care plans.
  • Coordinate and deliver 
    staff training
    , refreshers, and competency checks.
  • Maintain and monitor 
    compliance registers
     including:
  • Incident and Risk Registers
  • Training Register
  • Medication and Restrictive Practice Registers
  • Complaints and Compliments Register
  • Continuous Improvement Register
  • Ensure compliance with BSPs, WHS, infection control, and emergency procedures.
  • Liaise with RNs, BSP practitioners, support coordinators, and families to ensure consistent care.
  • Report and escalate
     operational risks, incidents, and service gaps to Directors in a timely and structured manner.
  • Contribute to quality assurance, internal audits, and continuous improvement projects.

What we're looking for

  1. Previous experience in a similar coordination or administrative role, within the disability services sector
  2. Strong organizational and time management skills with the ability to prioritize multiple tasks
  3. Excellent communication and interpersonal skills, with the ability to liaise effectively with clients, families and colleagues
  4. A genuine passion for supporting people with disabilities and a commitment to providing high-quality care
  5. Relevant qualifications in disability services, community services or a related field
  6. A current driver's license and willingness to travel within the local area
