About the Company
Choice Health and Wellbeing Group is seeking a Disability Service Coordinator to join us, with offices-based North of Perth. The ideal candidate will join our friendly organization on a Permanent full-time basis.
The Disability Service Coordinator is a crucial role within our organization, responsible for ensuring that our services meet the highest standards of quality and are in compliance with the requirements of the National Disability Insurance Scheme (NDIS).
Are you an experienced leader in the disability sector, passionate about delivering high-quality, person-centered supports? Choice Health and Wellbeing Group is seeking a skilled NDIS Disability Service Coordinator
to manage a portfolio of NDIS services.
This role is responsible for leading staff teams, ensuring compliance with NDIS Practice Standards, and supporting participants to achieve their goals in safe, supportive home environments.
Key Responsibilities:
- Lead, mentor, and supervise SIL support teams including recruitment, training, onboarding, supervision, and performance management.
- Oversee participant services: intake, assessments, planning, transition, and ongoing reviews.
- Conduct regular site visits to SIL homes, monitoring staff practice, participant engagement, HSE standards, and property maintenance needs.
- Ensure compliance with all NDIS Practice Standards, including incident reporting, staff training records, internal audits and risk assessments, restrictive practice reporting and audit readiness.
- Facilitate regular team meetings, building a culture of accountability, collaboration, continuous improvement, and professional practice.
- Work closely with families, guardians, support coordinators, and other stakeholders to ensure positive participant outcomes.
- Manage rostering and service delivery in collaboration with operations manager.
- Handle referrals, risk assessments, and ensure progress reporting on NDIS goals.
- Oversee incident and restrictive practice reporting through appropriate channels.
- Attend stakeholder meetings and develop resources for staff and service users.
- Willingness and ability to participate in an after-hours on-call roster.
About you:
- Relevant tertiary qualifications in Disability, Community Services, Human Services, or related field.
- Minimum 2 years' experience in a leadership role within the NDIS or community services sector.
- Strong knowledge of SIL services, NDIS Quality & Safeguards Framework, and compliance processes.
- Demonstrated experience in staff management, performance improvement, Audit, and HR processes.
- Excellent communication and stakeholder engagement skills.
- High-level organizational and time management skills, with attention to detail.
- Proficiency in Microsoft Office and client management systems.
- Ability to participate in a rotating on-call roster.
