Position Details

Primary Role: NDIS Support Coordinator

Work Location: Nambour, Sunshine Coast

Employment Type: Part time, 4 day work week

Salary Range: $35 – $43 p/h – subject to award rate

Qualification & Experience: Relevant Diploma or Degree AND/OR Minimum 2 year experience in the disability support sector

Duties

• Develop trusted, positive and professional relationships with participants and their support workers.
• Support individuals and their nominated stakeholders, to understand the supports in their plan and navigate the service system, identifying and accessing mainstream, community, informal and funded support
• Negotiate services and rates, in accordance with NDIS guidelines and an individuals budget
• Develop service agreements and create electronic service bookings
• Strengthen and enhance participants capacity to coordinate supports, self-direct and manage supports and participate in the community
• Be the key contact for service issues, complaints, major changes and plan reviews
• Case note activity, monitor individual outcomes and satisfaction with service provision and review expenditure
• Attend immediately to any complaints submitted by participants and work collaboratively with teammates to resolve any concerns and/or conflicts

Please provide details of your knowledge and experience in:

• Supporting NDIS participants to understand their plans and help them connect with services to improve their quality of life
• Client-centred practice
• Knowledge of service providers, support options, and community networks available for people with disabilities
• Applying a sense of cultural competence – being aware of cultural sensitivities and respecting cultural differences
