About the Role

At LifeFul, our Support Coordinators live and breathe our REALISTIC values. As a Support Coordinator you will:

• Support participants to understand and make the most of their NDIS plans, including budgets and funding categories.
• Connect participants with services that align with their goals, values, and support needs
• Build participants' capacity to manage appointments, budgeting, and decision-making independently
• Identify and respond to barriers or risks, including crisis situations and complex care needs
• Advocate for participant rights, ensuring their voice drives all support decisions
• Collaborate with carers, providers, and external systems (e.g. health, housing, justice)
• Maintain detailed, accurate case notes and track outcomes in line with NDIS requirements
• Use tools like Care View, and Toggl for documentation, timekeeping, and billing compliance
• Deliver person-centered, trauma-informed, and culturally safe services that meet best-practice standards

Qualifications and Experience:

• The Right to Work in Australia
• 18 Months experience as a Support Coordinator or significant experience in disability and 12 months experience as a Support Coordinator
• Certificate III or above in Individual Support, Disability Support or relevant qualification in social work, community services, or a related field (or equivalent experience).
• NDIS Worker Screening Check.
• Working With Children Check.
• First Aid and CPR Certificate.
• All required NDIS Commision Training Modules
• Full Driving License
• Comprehensive Car Insurance
