About the Role

Rock Support Services is looking for a NDIS Participant Coordinator to play a vital role in supporting our participants and ensuring high quality service delivery. You'll be responsible for interpreting NDIS plans, coordinating individual supports, and helping participants achieve their goals, such as increased social inclusion and independent living. This role involves working closely with our team to provide on-the-ground guidance and support, making sure our services align with the highest standards.

We are looking for an individual who has:

• Diploma or Certificate IV in Community Services or related field, or 2+ years of experience as a NDIS Coordinator.

• Experience with NDIS service delivery, compliance with Disability Services Standards, and managing complex behaviours.

• A strong passion for making a positive impact in participants' lives by working closely with them, their families, and representatives to help achieve the goals outlined in their Action Plan.

• Current Blue Card, Disability Screening, First Aid, and CPR certifications.

• Valid Driver's License with comprehensive insurance for personal vehicle use.

• Excellent written and verbal communication skills.

Key Responsibilities:

• Coordinate with service providers and supports, and effectively meeting client objectives, requirements and appointments.

• Work directly with participants and their families to implement and monitor support plans, working towards positive achievement of participant outcomes.

• Manage shift notes, case reviews, compliance reports and SIL reports for stakeholder's meetings and contributing to the continuous improvement of service delivery.

• Develop and facilitate community programs that promote social inclusion and daily living skills, while maintaining strong relationships with families and external stakeholders.

• Maintain a safe and secure environment by adhering to Workplace Health & Safety standards and reporting any hazards or incidents.
