About the Role:
As a Support Coordinator, you'll start with a small participant caseload while building your skills and confidence. As you grow in the role, you'll actively identify new participant opportunities and develop strong partnerships with Local Area Coordinators (LACs), community groups, and service providers to expand LifeFul's reach and influence.
You Will:
- Support participants to understand and implement their NDIS plans, turning goals into actionable outcomes.
- Build strong, strategic partnerships with LACs, local providers, and community organisations to enhance participant choice, access, and engagement.
- Coordinate services, arrange agreements, and ensure support is delivered effectively and efficiently.
- Monitor participant progress, track outcomes, and escalate risks or concerns promptly.
- Identify and convert organic growth opportunities into new participants and service pathways through trusted community networks.
- Maintain operational excellence, safeguarding, and 85% billable hours weekly while expanding your caseload.

- Partnerships with LACs, providers, and community groups strengthen LifeFul's presence and generate new participant opportunities.

What We're Looking For:
- Experience in case management, care coordination, or disability services (NDIS experience highly valued).
- Strong understanding of NDIS funding, participant rights, service navigation, and sector trends.
- Exceptional relationship-building, networking, and communication skills, especially in fostering LAC and community partnerships.
- Proficiency with Microsoft 365, CRM systems, and accurate case documentation.
- Commercial acumen and confidence to identify growth opportunities in the community.

Mandatory Requirements:
- Certificate III (or equivalent) in Individual Support / Disability Support.
- NDIS Worker Screening Check, Working with Children Check, First Aid & CPR Certificates.
- Valid Australian Driver's License and vehicle insurance.
- Completion of NDIS Quality & Safeguards training modules.
- Right to work in Australia.
