**Position Summary**:
As a support coordinator, you will be responsible in supporting participants to achieve their goals, maintain or increase independence and sustain their freedom of choice to the supports they wish to receive. You will be helping to support members of the community and is suited for an empathetic, genuine and reliable individual that can communicate effectively with culturally and linguistically diverse individuals.

**You will be responsible for**:

- Communicating effectively with a range of demographics as well as building and maintaining relationships
- Assessing for the required supports to best support participants with their daily activities, improved daily living and overall capacity
- Coordinating and implementing required supports for participants in terms of onboarding for services, service agreements, service bookings and overall management of the required supports
- Recording and monitoring participants progress towards their goal and report writing
- Adhering to all legislative requirements and NDIA reporting protocol
- Budget management / forecasting to ensure continuity of supports is delivered sustainably
- Previous experience in Support Coordination
- Expertise within the National Disability Insurance Scheme (NDIS) processes
- Ability to assess, respond and manage individual needs, as well as build rapport
- Excellent connections with service providers and allied health professionals within the local community.

**Job Type**: Part-time
Part-time hours: 30 per week

**Salary**: From $58,606.85 per year
