Ndis Support Coordinator

**Key Responsibilities**:
- **Plan Implementation**:
- Assist participants to understand their NDIS plan and the available supports.
- Connect participants with suitable service providers and supports.
- Negotiate service agreements and monitor service delivery.
- Review and adjust the plan as needed in collaboration with the participant.
- **Capacity Building**:
- Empower participants to make informed choices about their supports and services.
- Build participants' skills and confidence in managing their NDIS plan.
- Support participants to develop and maintain relationships with service providers.
- **Advocacy**:
- Advocate on behalf of participants to ensure their needs are met.
- Assist participants to resolve any issues or concerns with service providers or the NDIA.
- Promote the rights and interests of participants in the community.
- **Administration**:
- Maintain accurate and up-to-date records of participant plans and service agreements.
- Prepare reports and documentation as required.
- Participate in team meetings and professional development activities.
**Key Performance Indicators (KPIs)**
- **Participant Satisfaction**: High levels of participant satisfaction with the support coordination service.
- **Plan Implementation**: Effective implementation of NDIS plans, with participants receiving the supports they need to achieve their goals.
- **Capacity Building**: Participants demonstrate increased skills and confidence in managing their NDIS plan.
- **Advocacy**: Participants feel supported and empowered to advocate for their needs.
- **Administration**: Accurate and timely completion of administrative tasks.
**Limitations**:
- **Scope of Practice**: Support Coordinators are not qualified to provide clinical advice or direct support services. They must refer participants to appropriate professionals for any clinical or therapeutic needs.
- **Conflict of Interest**: Support Coordinators must avoid any conflicts of interest in their work. They cannot recommend services from providers with whom they have a personal or financial relationship.
- **NDIS Guidelines**: Support Coordinators must adhere to the NDIS Code of Conduct and all relevant NDIS policies and procedures.
- **Confidentiality**: Support Coordinators must maintain the confidentiality of participant information in accordance with privacy laws and regulations.
**Job Types**: Full-time, Part-time
Pay: $36.00 per hour
Expected hours: No less than 30 per week

- Bachelor Degree (required)
**Experience**:
- healthcare: 3 years (required)
