Location: Northern Suburbs, Perth WA

Able – iQ is looking for a part-time NDIS Support Coordinator to support participants across Perth's northern suburbs. This flexible work-from-home role focuses on coordinating services for people with complex support needs, especially those with psychosocial disabilities.

You'll work closely with participants, their families, and local providers to help them navigate the NDIS system, access the right supports, and build their independence. We're looking for someone confident working remotely who also knows the northern corridor well.

What you'll do:

  • Coordinate NDIS supports based on participant goals and funding
  • Refer and connect participants to local providers in the northern region
  • Monitor budgets and prepare reports for plan reviews
  • Facilitate stakeholder meetings and case discussions
  • Support participants through crisis or complex situations
  • Maintain clear, accurate documentation using Microsoft 365
  • Build strong working relationships across local networks

What we're looking for:

  • Minimum 3 years of Level 2 NDIS Support Coordination experience
  • Confident working independently and managing your own caseload
  • Strong knowledge of the NDIS framework and northern Perth service providers
  • Compassionate, culturally sensitive, and client-focused
  • Excellent verbal and written communication
  • Must have full working rights, valid NDIS Worker Screening & WWCC
  • Based in Perth's northern suburbs with a driver's licence and insured vehicle
