Key Responsibilities
- Demonstrating a solid understanding of NDIS principles, legislation, systems, and processes to ensure compliance and best practice in the support provided
- Demonstrated skills and understanding of planning, implementing, and evaluating plans
- Monitoring and managing the participants budget and coordination of plan
- Building knowledge of local services and organisations participants can utilise
- Engaging and coordinating service providers on behalf of the participants and linking participants to relevant providers
- Meet with and engage participants to implement their plans
- Engage participants informal support (family, carers etc.)
- Working with participants to help build their own capacity and independence, while ensuring choice and control
- Engage with NDIA staff and LAC's
- Assessing and managing risk in line with Soul Services WA's policies and procedures
- Achievement of key performance indicators
- Managing participant expectations, feedback and complaints
- Provide the NDIA with reports on outcomes and success indicators within the agreed reporting frequency
- Knowledge and understanding of NDIS Quality and Safeguarding and reporting
To be a successful candidate you will need:
Relevant experience in previous Support Coordination role with a minimum of 2 years' experience.
- Current Working with Children Check
- Current Police Clearance
- Current NDIS Worker Screening Check
- Valid driver's license and access to a registered roadworthy and insured vehicle (minimum 3rd party property)
- Quality and Safeguarding NDIS Worker Module
- Entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position
