About the Role
We are seeking a passionate and dedicated Support Coordinator to join our team. In this role, you will work alongside NDIS participants to strengthen their ability to understand, implement, and manage their NDIS plans. You will connect participants with the right supports, build capacity, and ensure services are delivered in line with their goals and aspirations.
Key Responsibilities
• Support participants to navigate the NDIS and build confidence in exercising choice and control.
• Develop, coordinate, and monitor supports in line with participants' NDIS plans.
• Engage with participants, families, service providers, and other stakeholders to ensure collaborative support.
• Resolve issues, manage crises, and provide advocacy where needed.
• Maintain accurate records, progress notes, and reporting in line with NDIS requirements.
About You
We are looking for someone who is empathetic, resourceful, and well-organised, with a strong understanding of the NDIS framework.
Experience working as a support coordinator/ LAC and have a sound understanding of the NDIS
Experience in disability, mental health, community services, or case management.
- Strong understanding of the NDIS and person-centred practice.
- Excellent communication, negotiation, and problem-solving skills.
- Ability to manage time effectively and prioritise competing demands.
- Have an empathetic, compassionate and professional working ethic
- Guarantee a participant focused approach
- Have the ability to work independently and with limited direct supervision in your day-to-day work
- Be trustworthy, respectful and open minded
- Exceptional attention to detail and record management skills
- Highly developed communication and interpersonal skills
Requirements
Experience as a support coordinator or Local area coordinator
Current NDIS Worker Screening Check (or willingness to obtain)
- Current Working with Children Check.
- National Police Clearance.
- Hold a current driver's licence and reliable vehicle.
- Relevant qualification in Community Services, Disability, Social Work, or related field (desirable).
- Computer literacy and record-keeping skills.
- Astalty experienced (desirable)
The successful candidate will be required to provide clear probity checks including a NDIS screening Check, Working with Children Check prior to commencing employment.
