NDIS Specialist Support Coordinator

Are you a Physiotherapist, Occupational Therapist or Registered Nurse seeking a change in pace? Looking to step away from the treatment room but still make a meaningful impact? Join our team as a Specialist Support Coordinator / Clinical Case Manager and take your career in an exciting new direction

SSCS WA is a leading provider of disability support coordination services and complex clinical case management, specialising in supporting individuals with complex clinical needs, justice interface and psychosocial disabilities. We have a large clientele base in metro and regional Western Australia and are passionate about ensuring that individuals across Australia receive gold standard of care and support without compromise.

What You'll Do:

  • Work closely with individuals with complex disabilities and their families to understand their needs, goals, and preferences.
  • Use your clinical expertise to assess needs and coordinate services.
  • Provide comprehensive support in navigating the NDIS system, including plan development, implementation, and review.
  • Conduct thorough assessments to identify strengths, challenges, and support requirements.
  • Collaborate with external service providers, community organisations, and government agencies to coordinate and access appropriate supports and services.
  • Empower clients to build their capacity for self-management and advocate for their rights and preferences within the NDIS framework.
  • Maintain accurate records and documentation in accordance with NDIS guidelines and organisational policies.
  • Stay informed about changes and updates to the NDIS framework and relevant legislation.

What We're Looking For:

  • Qualified physiotherapist, occupational therapist, registered nurse or social worker with current registration.
  • Strong communication and interpersonal skills, with the ability to build rapport and establish trust with clients and stakeholders.
  • Excellent organisational and time management skills, with the ability to prioritise tasks and manage competing priorities effectively.
  • Commitment to upholding the rights, dignity, and independence of individuals with disabilities.
  • Interest in complex clinical case management and willingness to learn.
  • Current driver's license and access to a reliable vehicle.
