About the role

This is an exciting opportunity to join Rhonda Jane' as a Level 2 Support Coordinator. As a key member of our team, you will play a vital role in supporting and empowering individuals with disabilities to live independently and achieve their personal goals. This part time/Full-time role is based in Warrnambool, VIC 3280, with the option for some remote work.

What you'll be doing

  • Assessing client needs and creating tailored support plans to address their unique requirements
  • Coordinating and monitoring the implementation of support services, ensuring high-quality care
  • Advocating on behalf of clients to secure appropriate funding and resources
  • Collaborating with a network of service providers, healthcare professionals and community organisations
  • Regularly reviewing and updating support plans to ensure they continue to meet client needs
  • Providing guidance, mentoring and support to more junior members of the team

What we're looking for

  • Relevant tertiary qualification in a field such as social work, occupational therapy or disability studies
  • Minimum 2 years' experience in a similar support coordination or case management role
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and stakeholders
  • Strong problem-solving and decision-making abilities, with a focus on delivering practical, client-centred solutions
  • Proficient in the use of relevant software and data management systems
  • Demonstrated understanding of the NDIS and disability support services landscape
  • Compassionate, empathetic and committed to making a positive difference in the lives of others
  • Current NDIS Worker Screening Check (or willingness to obtain)
  • Valid Working with Children Check
  • Current Australian driver's licence and access to reliable vehicle
  • Australian citizenship, permanent residency, or valid work visa
  • Current National Police Check (or willingness to obtain)
