EmpowerU is seeking an experienced Support Coordinator to join our growing team in the South West Sydent and Western Sydney area.

Key Responsibilities

With a focus on co-production, empowerment and capacity building, the Support Coordinator will work with participants and their support networks to:

  • Implement NDIS Plan's within the context of the NDIS framework, mainstream and community services
  • Identify appropriate services, and work with participants to achieve their goals
  • Support participants within a person-centred framework with a think outside the box attitude
  • Build and sustain strong collaborative relationships with government and non-government bodies, service providers, allied health professionals, and practitioners
  • Build participant's capacity to self-direct, self-advocate, work with professionals and other supports and manage their lives and their NDIS plan the way they choose
  • Maintain accurate and up to date case notes on progress via a client information system
  • Provide required NDIS reports, including progress reports and end of plan reports
  • Comply with all relevant legislation and codes of conduct
  • Assessing and managing risk in line with organisational policies and procedures
  • You will need to be able to work independently and be an active part of EmpowerU wellbeing teams.

  • Excellent communication (written and verbal) and interpersonal skills
  • Experience as a NDIS support coordinator or equivalent

  • Lived experience is highly valued

  • 100 Points of identification.
  • Full Australian working rights.
  • NDIS Worker Screening Check
  • Working with children check
  • A valid, unrestricted driver's licence and insured motor vehicle

Selection Criteria

  • At least 2-year experience as an NDIS support coordinator.
  • Sound working knowledge of the National Disability Insurance Scheme (NDIS), rules and operational guidelines, and experience in coordinating a range of community-based supports
  • Demonstrated ability to identify and provide a range of solutions to mitigate challenges, barriers, and risks with a focus on co-production with participants and stakeholders
  • Work within a person centred model
  • Excellent interpersonal skills that value individual and cultural diversity
  • A strong solution focus and the ability to think creatively outside of the box
