Your Story Community Supports (YSCS) is a registered NDIS provider with a team of over 140 staff across multiple states, and we are continuing to grow nationally. We deliver a wide range of supports including Community Participation, Supported Independent Living (SIL), and Specialist Disability Accommodation (SDA). Our mission is to provide safe, high-quality, and person-centred services that empower participants to achieve their goals.
About the Role
We are seeking a motivated and organised Community Participation (CP) Team Leader to lead our community programs and support worker teams. This role is responsible for overseeing the day-to-day operations of CP services, ensuring staff are supported, rosters are managed effectively, and participants are actively engaged in the community in line with their NDIS goals.
Key Responsibilities
- Lead, mentor, and support CP support workers to deliver high-quality, person-centred services.
- Develop and manage rosters, ensuring coverage for community activities and programs.
- Coordinate daily CP operations, including outings, group programs, and 1:1 supports.
- Build and maintain strong relationships with participants, families, and stakeholders.
- Ensure compliance with NDIS Practice Standards, safeguarding, and organisational policies.
- Oversee documentation including progress notes, incident reports, and risk assessments.
- Promote social inclusion, skill-building, and meaningful community participation for participants.
- Provide regular updates to management and contribute to continuous service improvement.
Skills & Experience
- Previous experience in disability, community services, or youth work (NDIS experience highly regarded).
- Strong leadership skills with experience supervising or coordinating staff.
- Knowledge of NDIS Practice Standards and community-based program delivery.
- Excellent communication and organisational skills.
- Ability to problem-solve and support staff in dynamic environments.
- Current NDIS Worker Screening Check, Working with Children Check, First Aid & CPR.
- Valid driver's licence and access to a reliable vehicle.
