We're searching for a
Senior Care Coordinator
who is passionate about leading with empathy, inspiring teams, and driving excellence in home care services.

What You'll Be Doing

  • Overseeing the delivery of person-centred care to our participants
  • Collaborating with Care Partners to develop care plans that truly reflect person-centred goals and needs
  • Collaborating with families, carers, and multidisciplinary teams
  • Managing rosters, schedules, and resources efficiently
  • Driving continuous improvement, compliance, and quality care outcomes
  • Leading with compassion while ensuring regulatory and safety standards are met

What You Bring

  • Certificate IV in Aged Care (or equivalent)
  • Strong experience managing Home Care Packages and coordinating service delivery
  • A natural ability to lead, inspire, and coordinate sub-contractor deliver
  • Excellent communication, conflict resolution, and organisational skills
  • High computer literacy (AlayaCare experience is a plus)
  • A genuine passion for the holistic well-being of the elderly and their families
