Here2Home provides NDIS Specialist Support Coordination, support coordination, social work, and psychosocial recovery coaching to participants of the National Disability Insurance Scheme (NDIS). Our services are designed to help individuals achieve their goals and improve their quality of life. Our team of dedicated professionals works diligently to ensure every participant receives tailored assistance that addresses their unique needs and circumstances.

Role Description

This is a sub-contracting role for an NDIS Support Coordinator (or Specialist). The Support Coordinator will be responsible for coordinating support services, facilitating communication between participants and service providers, report writing and delivering exceptional customer service. Day-to-day tasks include developing and managing participant support plans, connecting participants to services, tracking budgets, attending meetings, report writing, and case noting.

  • Positions are available in the North and South of Adelaide. Please do not apply if you are unwilling to travel to these locations to visit participants.
  • Minimum of 1 year experience as a support coordinator is required for this role. Do not apply if you don't meet this criteria.
  • Must have or be willing to obtain an NDIS Clearance and Working With Children Check.

Qualifications

  • Excellent in communication skills with people with disability and stakeholders
  • Proficiency in report writing
  • Ability to work independently and remotely
  • Willingness to conduct home visits to participants
  • Strong organisational and time management skills
  • Relevant experience in the disability or support sector
  • Bachelor's degree in Social Work, Psychology, or related field is essential if applying for the Specialist Support Coordinator role.
