About Herald Australia Pty Ltd Herald Australia Pty Ltd is a reputable provider of client-centered support services. We're looking for a motivated and capable Support Coordinator to join our team on a contracted basis and help deliver high-quality, coordinated support to our clients.

Role Overview We are seeking a proactive Support Coordinator to manage and coordinate client support services, acting as a primary liaison between clients, families, and service providers. This role requires excellent communication, strong organisational skills, and a commitment to delivering positive client outcomes.

Key Responsibilities

  • Coordinate and monitor client support plans and service delivery
  • Serve as the primary point of contact for clients, families, and external service providers
  • Schedule appointments, coordinate service delivery, and track progress and outcomes
  • Maintain accurate client records and documentation, ensuring privacy and compliance
  • Support onboarding of new clients and service providers
  • Prepare concise progress reports and communicate updates to management
  • Identify risks and issues, escalating promptly as needed
  • Contribute to ongoing improvements in service delivery and process efficiency

Requirements

  • Experience in support coordination, case management, or a NDIS related role
  • Exceptional communication, organizational, and problem-solving skills
  • Ability to work independently and manage multiple priorities
  • Proficiency with MS Office or Teams Workspace
  • Understanding of privacy and confidentiality requirements
  • Australian work rights (and current location within Australia or ability to work remotely as applicable)
  • NDIS WC and WWCC
  • Experience in disability, aged care, or allied health coordination
