About the role
The NDIS Care Coordinator will serve as the primary liaison between clients, their families, support coordinators, and service providers. This role involves coordinating and overseeing the implementation of participants' NDIS plans, ensuring that services are delivered effectively and in alignment with individual goals. This role will foster strong relationships with participants and stakeholders to promote optimal outcomes. This role will be based at our main office in Hampstead Gardens, SA. Full time and part time roles are available.
Responsibilities
- Serve as the primary contact for NDIS participants and their families regarding care coordination.
- Ensure excellence in participant service delivery, including goal planning and onboarding of new clients.
- Collaborate with participants to develop and implement support plans that align with their goals and needs.
- Assist in the development of agreements such as service agreements and accommodation agreements, and ensure they accurately reflect the supports being delivered.
- Manage participant needs and goals through assessments and care planning.
- Resolve participant escalations and complaints in a timely and effective manner as per the internal procedures.
- Develop exceptional relationships with both existing and new participants and their families.
- Investigate incident reports, resolve and escalate as necessary and update the incident report register.
- Check progress notes, liaise with support coordinators to provide feedback on participant care.
- Develop and maintain routine processes.
- Communicate, obtain, and record necessary services information.
- Ensure compliant documentation for clients as required by auditors.
- Attend NDIS client review meetings when necessary.
- Provide support to office staff as needed.
- Communicate with clients, employees, and stakeholders by phone or in person.
- Contribute to planning and participate in organisational events and activities.
Qualifications and Experience
- Bachelor's degree in a related field (Desirable)
- Demonstrated experience in a similar role within the NDIS or disability services sector.
- Strong understanding of the NDIS framework and person-centred planning principles.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients, families, and stakeholders.
- Proven ability to develop and implement individualised support plans.
- Strong organisational and time management skills, with attention to detail.
- Proficiency in using client management systems and data reporting programmes.
- Ability to work independently and as part of a multidisciplinary team.
