We're Hiring: Support Coordinator (Hybrid / Part-Time) – Melbourne

At Aligned Assist- complex care specialist — we're about connection, empowerment, and meaningful outcomes for every participant since 2019.

We're looking for an experienced Support Coordinator- Level, 2 and 3 with specialisation in the industry to join our growing team in Melbourne

Focus on emergency hospital discharge programs, complex care, plan reviews, SDA's/SIL/ILO is preferred as we specialise in these areas.

This part-time hybrid role (online meetings, home visits, scheduled team meetings, WFH) is perfect for someone passionate about helping NDIS participants achieve their goals while enjoying work-life balance and flexibility with a team together on a mission Aligned to grow together.

What You'll Brin

g: Experience in Support Coordination or the NDIS sector (Minimum 2 year

s) Strong communication and organisational skil

ls Empathy, problem-solving, and a participant-focused minds

et Knowledge of the NDIS and local community suppor

ts Qualification in Community Services, Disability, or related field (preferre

d) Leadership qualities and compassionate to support individuals who need an advocate working together as a team to achieve goals
