Specialist Support Coordinator

Code Black Psychology is a specialised multidisciplinary organisation consisting of psychologists, social workers, allied health professionals, and legal practitioners. Through teamwork we provide complex case consultations, diagnostic reviews, care team support and coordination, behaviour support and change, NDIS specialist support coordination, bespoke training and advice on policy and framework development.

Our Specialist Support Coordinators offer NDIS support coordination tailored to individuals facing complex challenges, including persistent risks to themselves and others. Our clients may exhibit one or more of the following characteristics:

  • High-risk, high-harm behaviours and involvement with the criminal justice system.
  • Multiple and dual diagnoses with intricate healthcare and support requirements.
  • Significant health, welfare, and community-based service needs marked by engagement challenges and limited participation with available supports.
  • Multiple vulnerabilities such as multiple disabilities, a history of hospitalisations and incarcerations
  • A disconnection from culture, community, and identity.

Specialist Support Coordinator Responsibilities

  • Manage complex caseloads and specialise in Level 3 support coordination, maximising NDIS plan implementation for participants.
  • Execute specialist support coordination activities, including crisis planning and addressing barriers.
  • Provide reports to NDIA on participant capacity building impact.
  • Coordinate care team requirements for participants.
  • Maintain accurate records and communication.
  • Encourage service innovation, continuous improvement, and a culture of best practices.
  • Contribute to new service development as needed.
  • Report any risks arising in program provision to Code Black Team Leader and Manager.

To excel in this position, you will possess;

  • Proven expertise in tailoring individualised support for individuals facing complex needs.
  • Deep appreciation for individuals with disabilities and their families, reflecting high ethical and professional standards.
  • Comprehensive operational knowledge of the NDIS and relevant legislation, policies, regulations, and service benchmarks.
  • Commitment to person-centred and family-centred practice.
  • Proficient leadership skills and the capacity for effective organisation.
  • Proactive and confident in approach to work with high standards of ethical and professional behaviour.
  • A desire to collaborate with stakeholders to enhance client capacity and well-being, with a focus on monitoring and reporting outcomes.
  • Innate curiosity, a passion for learning, and a readiness to tackle challenges.
  • Embracing cultural awareness and inclusivity as integral components of professional practice.

This position is full-time, with the possibility of negotiating a 0.8 FTE agreement. We advocate for a balanced approach to remote and in-person work, with our office situated in the Malvern area. Travel within Victoria, including occasional visits to rural and regional areas, may be required as part of the role.
