The Opportunity
We're looking for an experienced and passionate Nurse Practitioner to provide short- term, wraparound care for aged care residents with limited GP access.
This is a unique opportunity to combine Telehealth flexibility with occasional outreach visits — delivering meaningful impact across the Sydney Metro region.
Full-time or part-time options are available. The role is primarily Telehealth-based, supported by a $200 weekly travel allowance for any in-person visits.
Your Role
- Conduct comprehensive clinical assessments, diagnoses, and treatment planning
- Deliver care primarily via Telehealth, with occasional in-person visits
- Collaborate closely with GPs, allied health professionals, and care teams
- Use digital health tools to enhance continuity and care outcomes

About You
- Endorsed Nurse Practitioner with strong clinical capability and autonomy
- Experience in aged care, chronic disease management, or primary care
- Excellent communication and collaboration skills
- Passionate about improving access and outcomes for older Australians
