Who We're Looking For

We're seeking 
experienced Support Coordinators
 who specialise in the participants others find challenging, those navigating complex mental health, hospital to community transitions, housing instability, and psychosocial disability.

If you're the coordinator that hospital discharge planners, community mental health teams, and AOD services call when they need someone who actually gets results, we'd love to talk.

What Makes You Stand Out

You bring experience in:

  • Psychosocial disability, complex mental health presentations, or hospital discharge coordination
  • Working with participants who have dual diagnosis, trauma history, or have experienced multiple service failures
  • Coordinating across fragmented systems (NDIS, health, housing, justice) to create sustainable outcomes
  • Building therapeutic rapport with participants others have struggled to engage

You're known for:

  • Having established relationships with psychiatric services, NGO housing providers, AOD programs, or community mental health teams
  • Your clinical maturity, you work autonomously but know when to collaborate or escalate
  • Actually achieving outcomes with high-complexity, high-risk participants
  • Being the coordinator referral partners trust and participants stick with

What You'll Do

  • Provide high, complexity coordination for participants with psychosocial disability and co-occurring challenges
  • Coordinate hospital to community transitions, working directly with discharge planners and inpatient teams
  • Build strategic partnerships with NGO housing, mental health services, AOD programs, and community organisations
  • Advocate systemically, ensuring mainstream services fulfill their obligations before NDIS funding becomes the default
  • Monitor plan utilization with a focus on capacity building and participant self-determination
  • Maintain clear, clinically informed documentation that meets NDIS Practice Standards

What You'll Need

The basics:

  • ABN and insurances (Public Liability + Professional Indemnity)
  • Valid NDIS Worker Screening Check
  • Working With Children Check (Queensland)
  • Current driver's licence and insured vehicle
  • Relevant qualifications (Social Work, Psychology, OT, Mental Health, Community Services, or equivalent)

The important stuff:

  • Demonstrated experience in complex support coordination, case management, or mental health support
  • Professional networks within mental health, hospital, housing, or AOD sectors
  • Strong understanding of trauma-informed practice and psychosocial disability
  • Ability to communicate confidently with psychiatrists, allied health professionals, and multidisciplinary teams
  • Resilience, adaptability, and genuine passion for this work
