About The Role:

The position is based in Tamworth NSW, with some required travel within the New England region.

As a Support Coordinator, you will strive for excellence to support people through a strength-based person-centred way to live the life they choose. You will focus on building capacity, doing with not for. You will be committed in supporting people with a disability to have choice, opportunity, inclusion, and acceptance in the community.

Some core functions of the position include:

  • Assist people with a NDIS plan to achieve their goals.
  • Assist in managing supports to best utilise NDIS funding.
  • Support to connect with services that meets individual needs.
  • Coordinate services to ensure collaboration and consistent supports.
  • Support the NDIS planning process and reviews.

About You:

You will need to be a creative, passionate individual, that is committed to professionalism, teamwork, diversity, and inclusion, engaging with others and provide excellent customer service.

To be considered for this position, you must demonstrate:

  • Experience working within the community sector, supporting people with disability, complex needs, and mental health.
  • Have experience in case management and/or care coordination.
  • Demonstrated experience in the provision of client directed strength-based approach and person-centred services.
  • Demonstrated ability to develop professional networks, and stakeholder engagement.
  • Excellent time management skills and ability to prioritise tasks and competing demands.
  • Demonstrated interpersonal skills inclusive of verbal and written communication skills.
  • Demonstrated experience in the use of risk assessments.
  • Strong computer literacy.
  • Strong report writing and budgeting skills.

You must possess/be willing to obtain before the commencement of employment:

  • Have full working rights within Australia.
  • Working with Children Check.
  • NDIS worker screening clearance.
  • Driver's License.
  • Completed NDIS Worker Orientation "Quality, Safety and You" module.

Desirable:

  • Demonstrated experience supporting people with mental health.
  • Certificate IV in Disability, Mental Health, or related discipline.
  • Demonstrated understanding of NDIS funding, individualised funding. approach and NDIS frameworks.
