Employment Type: Full-time or Part-time

Classification: SCHADS Level 4.1–5.1

Salary: $88,099 – $100, % super (annual review)

About Heart Worx

Heart Worx provides Support Coordination, Psychosocial Recovery Coaching and direct support across the Tamworth region.

We're a small, lived-experience-led organisation built on clear communication, realistic workloads and person-led practice.

Our team includes coordinators and recovery coaches who've personally navigated the NDIS, mental health systems or caring roles – we use that insight to keep our work grounded and human.

Our focus is helping people use their NDIS plans well and build steadier lives through practical, coordinated support.

This role centres on planning, communication and advocacy, not direct care or therapy.

The Role

You'll help participants use their NDIS plans effectively by:

  • coordinating supports and communicating with providers, plan managers and clinicians
  • tracking actions, budgets and outcomes through clear documentation
  • maintaining realistic caseloads and structured workflows
  • building your knowledge of NDIS systems and reporting through regular supervision

You'll start with supported onboarding and shadowing, moving toward an independent caseload when ready.

This role suits people who enjoy structure, consistency and follow-through – not just helping in the moment.

It's a good fit for curious, organised thinkers who like solving real-world problems and seeing steady progress over time.

What You'll Bring
  • Strong writing and communication skills
  • Confidence with email, systems and detail management
  • A calm, reliable presence and sound judgement
  • Curiosity and willingness to learn complex systems
  • Interest in mental health, disability or community work
  • Some experience or volunteering in mental health, disability or community services is preferred — e.g. support work, case work, employment services, youth or housing roles, or team leadership
  • Current NDIS Worker Screening, WWCC and Driver Licence (or ability to obtain)

What We Offer
  • Structured onboarding and paid training
  • Regular supervision and mentoring with experienced leadership
  • Realistic caseloads – no meaningless KPIs
  • Hybrid work model (community and home-based)
  • Templates, tools and admin systems that make coordination sustainable
  • A grounded, supportive team culture that values steadiness and follow-through
  • Pay and role progression linked to skill development, not burnout

How to Apply

Apply directly through SEEK.

Shortlisted applicants will be contacted for a brief phone chat, followed by a full interview at our Peel Street office.

We know coordination can be tough work – but it's easier when you're in a team that talks, listens, and backs you up.

If you're ready to build a sustainable, skill-based career in disability support, we'd love to hear from you.
