About the Role
We're looking for an experienced and compassionate Coordinator of Supports to join our growing Tamworth team.
In this rewarding position, you'll work directly with NDIS participants to coordinate their supports, connect them with services, and build their confidence to live more independently. You'll also play a key role in mentoring others, managing complex coordination cases, and driving best-practice outcomes for participants.
Key Responsibilities:
Support participants to understand, implement, and manage their NDIS plans.
Connect participants to community, mainstream, and funded supports.
Coordinate service delivery and maintain strong provider and stakeholder relationships.
Empower participants to develop confidence, independence, and self-direction.
Monitor participant progress and assist with plan reviews.
Maintain accurate and timely documentation and case notes.
Collaborate with the wider Sage team to deliver high-quality, person-centred services.
This role is primarily community-based, with flexibility to work from home and the office as needed.

About You
You're compassionate, motivated, and committed to helping people with disability live their best lives. You bring strong NDIS knowledge, excellent communication skills, and a genuine passion for making a difference.
Essential Criteria:
Experience as a Support Coordinator (or equivalent NDIS role).
Minimum Diploma in Community Services, Disability, Mental Health, Social Work, or related field.
Strong organisational and administrative skills.
Current Driver's Licence and reliable vehicle.
Valid NDIS Worker Screening Check and Working with Children Check (or willingness to obtain).
Completion of the NDIS Worker Orientation Module: "Quality, Safety and You."
Desirable:
Knowledge of Tamworth and surrounding community/disability networks.
Experience supporting participants with psychosocial or complex needs.
